Online Nominations

1. All Cultural Medallion and Young Artist Award nominations have to be made online by Friday, 30 March 2012, 6pm.
   
   
2. In filling in the nomination, you will be requested to provide:
   • Your Contact Details
   • Your Nominee's Personal and Contact Details (e.g., NRIC number and date of birth)
   • Basis for Your Nomination (e.g., Nominee's CV and other supporting documents)
     
     
3. After submitting the online nomination form, please print a copy the completed nomination form.
   
   
4. A hardcopy version of the online nomination form with Section D - Declaration signed by both nominator and nominee should be submitted alongside the nominee's portfolio and other supporting documents to the following address:

5. Portfolios and supporting documents postmarked or received by hand later than Monday 9 April 2012, 6pm, will not be considered.

Portfolio Guidelines

1. Your Portfolio Submission should include only 2 sets of the nominee's most recent and best works. You are advised to compile excerpts and footages of the nominee's works in a digitised format (CDs/DVDs/VCDs). Depending on the nature of the nominee's work, these may comprise:
   
   • Audio Recordings
   • Video Recordings
   • Photographs
   • Digitised Publications and Catalogues
     
     
2. The portfolio submission may also include hard copy materials that cannot be digitised, such as:
   
   • Excerpts of Publications and Catalogues
   • Musical Scores
   Note: In the case of composers and conductors, copies of music compositions and video footage showing conducting of performances respectively are necessary.
   
   
3. In your submission, please include critical essays, press reviews, articles, letters of support and any other documents that would assist in the assessment.
    
4. Your portfolio submission will be retained by the National Arts Council until the Arts Awards Presentation Ceremony in October 2012. After the ceremony, we will arrange a time for you to collect the submitted materials from the National Arts Council.
